Fax orders are accepted 24 hours/7 days at (760) 727-6332
Specify the details such as quantity, force level, cushion style, size and color.
If you specify your fax number, we will send you a proforma invoice prior to shipment to verify your order.
Freight and handling charges as well as appropriate sales taxes and
customs fees and duty will be added to the product cost to determine
the total invoiced on the order.
Shipment will be made via best way unless otherwise specified.
Payment Purchases may be paid by American Express, VISA, MasterCard, wire transfer or COD. Bank wire transfer information available upon request.
Terms All products are for single patient use only. Ortho Kinetics
Corporation will not be responsible for for claims arising from
improper use or misuse of products. Please follow the product's
instructions for proper use indications. Orthodontic products are
medical products available by prescription to professionals.
Merchandise may be returned for credit within 30 days from date of
purchase. A conditional warrantee against defects in manufacturing and
workmanship will be extended for one year from date of purchase.
To return products, call for a return authorization request number
(RMA). Returns must be accompanied with a proper return authorization
number. Any product in the original packaging, unopened and unused may
be returned within 30 days. All returns are subject to a 15% restocking
and inspection fee.
