Headgear Wear Scorecard

We developed  a headgear wear score card that can be printed out from your printer.  It allows patients to keep track of hours of daily headgear wear. 

Here is a sample of the chart:

Your child colors in each box to reflect # hours of wear for that night.

The chart is made in Microsoft Excel.  You will need Excel to be loaded on your computer  to view, print or modify the chart.

It is a Microsoft  Excel spreadsheet.  There are two versions:  One scaled for 10-24 hours per day of wear, and one scaled for 3-18 hours per day of headgear wear (more motivational). I use the 3-18 hour version of the chart with my daughter.

Parents can use this chart with their children to see how their child's headgear compliance is going.  You can set rewards for good performance.

We encourage orthodontic offices to save a copy of these and add their office's name, Logo and such to the bottom of the page.
